Spatial disorientation is the inability of a pilot to   correctly interpret aircraft attitude, altitude, or airspeed in relation to the Earth or other points of reference. It can be caused by flight instruments malfunction or low visibility.
The best way to avoid it is by reading, and following serviceable flight and navigation instruments.
