Unexpected Situation – Test 22

Welcome to the test Unexpected Situations – 22.

This test contains 3 situations that you can choose from the menu on the left hand side of this page. Always try to answer before reading the expected answer!

PILOT: AMSTERDAM TOWER, ALL NIPPON 67, WE REJECTED TAKE OFF DUE TO BODY GEAR STEERING ISSUES, REQUEST TO RETURN TO THE APRON. 

ATC: ALL NIPPON 67, ROGER, YOU ARE CLEAR TO VACATE THE RUNWAY VIA TAXIWAY WHISKEY 7 AND BRAVO, PLEASE INFORM US IF YOU NEED ANY ASSISTANCE.

EXAMINER: Now tell me in your own words everything the pilot and controller said.

The pilot informed the controller that they rejected the takeoff due to body gear steering issues and requested to return to the apron.

The controller cleared the pilot to return to the apron via taxiway whiskey 07 and bravo. The controller also asked the pilot if they needed assistance.

EXAMINER: What are the outcomes of body gear steering failure?

From my standpoint, if you have problems with body gear steering, you won’t be able to perform a 180 degrees rotation, obstructing the runway. Consequently, it will cause flight delays and flight cancellations as well.

EXAMINER: Now listen to situation number 2.

PILOT: PAN PAN, PAN PAN, PAN PAN, BRUSSELS APPROACH, WEST ATLANTIC 14, WE HEARD A LARGE ELECTRICAL CLUNK FOLLOWED BY THE LOSS OF PRIMARY EFIS SCREENS ON THE LEFT-HAND SIDE OF THE COCKPIT AND THE DISCONNECTION OF THE AUTOPILOT AND AUTO THROTTLE. REQUEST PRIORITY TO LAND ON RUNWAY 25 R.

ATC: WEST ATLANTIC 14, ROGER. WE ADVISE THAT YOUR TRANSPONDER SECONDARY RETURNS HAD CEASED. I WILL GIVE YOU PRIORITY TO LAND ON RUNWAY 25 R.

PILOT: BRUSSELS APPROACH, WEST ATLANTIC 14, WE ARE UPGRADING OUR CALL TO MAYDAY, REQUEST IMMEDIATE VECTORS FOR AN ILS APPROACH TO RUNWAY 25 R DUE TO AN ELECTRICAL SYSTEM FAILURE.

ATC: WEST ATLANTIC 14, ROGER MAYDAY. FLY THROUGH THE LOCALIZER. YOU ARE CLEAR FOR VISUAL APPROACH RUNWAY 25 R, WIND 230 DEGREES AT 5 TO 8 KNOTS. 

EXAMINER: Now tell me in your own words everything the pilot and controller said.

The pilot informed the controller that they heard a large electrical clunk followed by the loss of primary EFIS screens on the left-hand side of the cockpit and the disconnection of autopilot and autothrottle. The pilot then requested priority to land on runway 25 right.

The controller advised the pilot that their transponder secondary returns had stopped, and informed that priority to land would be given to runway 25 right.

The pilot decided to upgrade their call to a mayday call and requested immediate vectors for an ILS approach to runway 25 right due to an electrical system failure.

The controller cleared the pilot to fly through the localizer and to perform a visual approach to runway 25 right.

EXAMINER: What are the dangers of an electrical system failure?

The dangers of an electrical system failure are communication losselectrical fire and in the worst case scenario, a disaster. 

EXAMINER: Now situation number 3.

PILOT: PAN PAN, PAN PAN, PAN PAN, NEW ORLEANS CENTER, UNITED 430, WE HAVE A MEDICAL SITUATION ONBOARD, a coffee pot fell off a galley cart and spilled over a passenger causing SEVERE burns to the passenger. REQUEST TO DIVERT TO CLINTON NATIONAL AIRPORT AND MEDICAL ASSISTANCE UPON ARRIVAL. 

ATC: UNITED 430, ROGER YOUR URGENCY, YOU HAVE PRIORITY TO LAND AT CLINTON NATIONAL AIRPORT, RUNWAY 22 L. MEDICAL SERVICES ARE ON STAND BY.  

EXAMINER: Now tell me in your own words everything the pilot and controller said.

The pilot declared an urgency informing there was a medical situation onboard. The pilot told the controller that a coffee pot had fallen off a galley cart and spilled all over the passenger, causing severe burns to that passenger. They requested to divert to Clinton national airport and medical assistance upon arrival.

The controller acknowledged their urgency and informed the pilot that they had priority to land at Clinton national airport on runway 22 left, and informed that medical services were on stand by.

EXAMINER: What are the outcomes of a medical situation onboard?

 

The outcomes could be not only a flight diversion but also panic onboard.

EXAMINER: Now, considering the three situations you have heard in part three: How would you compare them? You may want to compare them in terms of severity, possible solutions and ways of prevention. Which one do you think is the most difficult to deal with? Why?

IN MY OPINION, SITUATION NUMBER ONE IS THE LEAST SERIOUS BECAUSE THE AIRCRAFT IS ON THE GROUND AND ALL PASSENGERS WERE SAFE. THE ONLY PROBLEM IS THAT ALL PASSENGERS WILL HAVE TO BE RELOCATED TO ANOTHER AIRCRAFT CONSEQUENTLY  THIS MIGHT CAUSE FLIGHT DELAYS AND FLIGHT CANCELLATIONS.

I BELIEVE, SITUATION NUMBER THREE IS WORSE THAN SITUATION NUMBER ONE. AS THERE IS A PASSENGER IN PAIN ONBOARD, TO DEAL WITH THIS THE PILOTS MUST LAND AS SOON AS POSSIBLE AND THIS WOULD CERTAINLY CAUSE FLIGHT DELAYS OR CANCELATIONS.

HOWEVER, SITUATION NUMBER TWO IS THE WORST SITUATION TO DEAL WITH BECAUSE THE PILOTS LOST THE FLIGHT CONTROLS ON ONE SIDE OF THE COCKPIT, AND DECLARED AN EMERGENCY.
THIS COULD CAUSE  CASUALTIES AND PANIC ONBOARD. IF I WERE THE PILOT IN THIS SITUATION, I WOULD PERFORM THE CHECKLIST AND DECLARE AN URGENCY OR AN EMERGENCY IF NECESSARY. THE BEST WAY OF PREVENTION IS REGULAR MAINTENANCE.
